The cycle starts when you activate the sim. The sim is activated the moment you order. I use the free service and like it so far... but no MMS, no voicemail (although people can leave you voicemail, you have to pay to get it), and included voice minutes is low quality that no one can hear or understand what you say. You can pay more to have *clearer* voice. Using the free data, I call through Whatsapp or Skype so the voice is a lot clearer.
If the minutes refresh every month than this is a great price.
Yes, the minutes, text and data all reset after the cycle ends... the cycle begins when you get the sim and ends 30 days later after you get the sim.

Yes and no. Freedompop used Sprint originally, then starting using T-Mobile and ATT. I'm currently using the ATT service and it does well for me. I get the free 200 minutes, 500 texts, 200 mb because I use a lot of wifi and I'm not a talker.
This is a good price if you want 5x more data. For me, free is pretty great.
Freedompop uses Sprint CDMA network. Here is the proof from their website:

Open a google voice account and set it up to forward to FreedomPop. And go into FreedomPop settings on the website and disable to VoiceMail completely. After that the Google Voice will be your voicemail. And of course you can send/receive MMS on Google voice with hangouts
